About the Plan
The Community Health Improvement Plan is a systematic, long-term effort to address key public health needs and issues in a community.
Wise Health System has developed the Community Health Improvement Plan, with collaboration from community stakeholders, to define and set priorities for a vision for Wise County’s community health. It contains goals, objectives, and strategies to improve the health of Wise County.
The Community Health Needs Assessment
The CHI Plan based on the results of the Community Health Needs Assessment, through comprehensive data collection and analysis.
The purpose of the assessment is to understand the needs unique to those in the community served by Wise Health System and to document compliance with federal laws.
As part of the Affordable Care Act, non-profit hospitals must complete a community health needs assessment every three years. Hospitals must collect input from the community, adopt a plan to address community needs and then report on these activities.
